  1. Power the phone on
  2. Enter ##000000#
  3. Select Edit click OK
  4. Enter MDN (10 digit number) click OK
  5. Enter MIN (10 digit number) click OK
  6. Select Done click OK
  7. The phone will power cycle and programming is now complete
